29 CFR 1910.213(a)(12): The portion of circular saw(s) beneath or behind the table, where there was a possibility of contact with the saw, was not covered with a guard or exhaust hood:     On or about July 30, 2013, the employer did not ensure the Hitachi Miter Saw located in the assembly area was properly guarded.  This condition exposes employees to an amputation hazard.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    On or about July 30, 2013 and times prior to, the employer did not ensure that employees doing maintenance on equipment in the shop were trained in Lockout Tagout procedures.  This condition exposes employees to an amputation hazard.
