29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:       1) On August 7, 2013, E Esquivel Roofing LLC did not protect its employees from falls in excess of 7 feet while engaged in a re-roof job.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder did not extend at least 3 feet above the upper landing surface; or where this was not possible due to the ladders length,  the ladder was not secured at its top to a rigid support:     1) On August 7, 2013, E Esquivel Roofing LLC did not protect its employees from falls in excess of 7 feet while accessing the roof by a ladder.  The ladder was not extended at least 3 feet above the eave, nor was it secured.
