29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about August 9, 2013, at 2863 Kelley Chappel Road, Decatur, Georgia: An employee performing masonry work was exposed to an electric shock hazard when he used a 120 volt, 7.5 amp Bosch Chipping hammer connected to a GFCI temporary power station that was not grounded.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about August 9, 2013, at 2863 Kelley Chapel Road, Decatur, Georgia: An employee performing masonry work from an approximately 5-foot by 7-foot tubular welded frame scaffold was exposed to a fall hazard of approximately 10-feet when he climbed the scaffold frame to access and egress the work platform because a ladder was not installed.
