29 CFR 1910.252(a)(2)(vi) Special Precautions and Prohibited Areas Cutting or welding shall not be permitted in the presence of explosive atmospheres (mixtures of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that may develop inside uncleaned or improperly prepared tanks or equipment which have previously contained such materials, or that may develop in areas with an accumulation of combustible dusts: On August 6th, 2013 at the jobsite located at 3651 E Kearny Street in Springfield, MO an employee was exposed to a struck by hazard in that the tank he was welding contained an explosive atmosphere due to improper preparation of the tank.

29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substance: On August 6th, 2013 at the jobsite located at 3651 E Kearny Street in Springfield, MO an employee was exposed to a struck by hazard in that the tank he was welding was not cleaned thoroughly to remove all flammable materials from the tank.
