29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On 12 August 2013, a worker was using a hand-held circular saw to cut pieces of wood on the ground without the use of eye protection, exposing the worker to a struck by hazard to the face and eyes from flying objects or debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On 12 August 2013, a worker was performing framing activities on a condominium under construction at the second story roof level along the outer top plate without fall protection, exposing the worker to a fall hazard of approximately 19 feet to the ground.

29 CFR 1926.1053(a)(3)(ii): Rungs, cleats, and steps of step stools shall be not less than 8 inches (20 cm) apart, nor more than 12 inches (31 cm) apart, as measured between center lines of the rungs, cleats, and steps. On 12 August 2013, a framing worker was accessing the upper portion of the second story roof of a condominium under construction by climbing up the frames bracing which was spaced at approximately 30 inches apart, exposing the worker to a fall hazard of approximately 10 feet to each level below.
