29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: (a) 2808 Highland Drive, Billings, Montana: On or about August 13, 2013 and times prior, an employee was exposed to a catapult and subsequent fall of approximately seventy-five feet to the ground below while working in an articulating boom lift without being secured.
