29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:      (a)  Construction Site: On or about August 13, 2013, and at times prior, employees were exposed to a fall of approximately sixteen feet to the ground below while tearing of existing asphalt shingles on a roof with a 6:12 pitch. The roof was being prepared for new asphalt shingle installation.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:    (a) Construction Site: On or about August 13, 2013, and at times prior, employees were exposed to a fall of approximately sixteen feet to the concrete below while accessing the roof on a ladder that extended approximately seventeen inches above the access level.
