29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   (a) Roof, new residential construction located at 1713 Whitfield Park Circle, Smyrna, GA 30080: Employees were exposed to eye injuries while operating pneumatic Hitachi (Model NV45AB2) Coil Roofing Nail Guns without eye protection. Employees firing 1 1/4-inch metal collated roofing nails at 110 PSI were operating these tools without safety glasses or goggles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) Roof, new residential construction located at 1713 Whitfield Park Circle, Smyrna, GA 30080: Employees were exposed to falls while engaged in residential roofing activities on a 6/12 pitched roof at heights of approximately 21-feet, 7-inches when they failed to connect to the fall protection system provided.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Rear side, new residential construction located at 1713 Whitfield Park Circle, Smyrna, GA 30080: The ladder provided was not secured to the structure and did not extend 3 feet above the working surface to allow for safe access on and off the roof. Employees accessing the ladder from the roof were exposed to falls of approximately 21-feet, 7-inches to the lower level.
