29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :    On or about August 21, 2013, the employer, Locknut Technology Inc., did not obtain a new audiogram for an employee who works on the south wall of the production area and is exposed at or above an 8-hour time-weighted average of 85 decibels      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program:  On or about August 21, 2013, the employer, Locknut Technology Inc., did not annually retrain an employee who works on the south wall of the production area who is included in the hearing conservation program.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  The employer, Locknut Technology Inc., did not conduct an initial noise exposure assessment prior to implementing a hearing conservation program.    1) An employee working at a machine on the south wall of the production area was exposed to continuous noise levels at 101% of the permissible daily noise exposure (8-hour time-weighted average sound level of 80 dBA)  or the equivalent sound level of approximately 90 dBA during the 464 minute sampling period on August 21, 2013; exposure calculations included a zero increment for the 16 minutes not sampled.    2) An employee working at a machine on the south wall of the production area was exposed to continuous noise levels at 92.2% of the permissible daily noise exposure (8-hour time-weighted average sound level of 80 dBA)  or the equivalent sound level of approximately 89.4 dBA during the 453 minute sampling period on August 21, 2013; exposure calculations included a zero increment for the 27 minutes not sampled.    3) An employee working at a machine on the south wall of the production area was exposed to continuous noise levels at 94.7% of the permissible daily noise exposure (8-hour time-weighted average sound level of 80 dBA)  or the equivalent sound level of approximately 89.6 dBA during the 466 minute sampling period on August 21, 2013; exposure calculations included a zero increment for the 14 minutes not sampled.   There is no abatement certification or documentation required for this item.
