29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) on all open sides:  On or about August 19, 2013, the employer did not ensure that the work platform on the Dragon XJ400 work over rig had guardrails installed on all sides, exposing employees to a fall hazard of approximately 4 feet.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about August, the employer did not ensure that the flexible cord supplying power to the 24 Volt  hydraulic pump motor control box were connected so that tension was not transmitted to the joints or terminals, exposing the employees to electrical hazards.
