29 CFR 1926.351(b)(1): Arc welding or cutting cable(s) was not of the completely insulated, flexible type, capable of handling the maximum current requirements of the work in progress.      On or about August 14, 2013, and times prior thereto, an employee was using a Lincoln Electric Ranger 250 GXT welder which had wires exposed on the electrode cable at the boot connection, exposing the employee to the hazard of electric shock.

29 CFR 1926.351(b)(2): Welding cable(s) in use was not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.     On or about August 14, 2013, and at times prior thereto, an employee was using a Lincoln Electric Ranger 250 GXT welder which had repairs to the cable within 10 feet of the electrode holder, exposing the employee to the hazard of electric shock.

29 CFR 1926.352(d): Fire extinguishing equipment available in work area(s), where welding, cutting, or heating was being performed, was not maintained in a state of readiness for instant use.     On or about August 14, 2013, and times prior thereto, there was no identifying tag or record of inspection for the fire extinguisher located on the Skyjack SJ111 scissor lift being used by an employee performing welding activities.  This exposed the employee to hazards associated with fire.    OR IN THE ALTERNATIVE...    29 CFR 1926.150(c)(1)(viii):  Portable fire extinguishers will be inspected periodically and maintained in accordance with Maintenance and Use of Portable Fire Extinguishers, NFPA No. 10A-1970.
