29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  Employees entering feed mixing wagons to make adjustments were exposed to entanglement hazards in that; the hydraulic motor attached to the pto shaft of the mixer was not de-energized and locked out to prevent the equipment from starting when the employees are inside the machine.
