29 CFR 1910.132(h)(1): Except as provided by paragraphs (h)(2) through (h)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, shall be provided by the employer at no cost to employees.  On or about August 08, 2013 and times prior thereto, the employer did not provide eye protection at no cost.  Employees were exposed to eye injuries when operating various types of lawn care equipment.

29 CFR 1910.141(b)(1)(i): Potable water was not provided in all places of employment, for drinking, washing of the person, cooking, washing of foods, washing of cooking or eating utensils, washing of food preparation or processing premises, and personal service rooms.   On or about August 08, 2013 and times prior thereto, the employer did not provide drinking water for employees performing lawn care activities. Employees were exposed to the hazard of dehydration.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about August 08, 2013 and times prior thereto, the employer has not performed a workplace hazard assessment.  Employees were exposed to eye and skin injuries when operating various types of lawn care equipment.
