29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: a) At the site, on or about 08/15/2013, a tubular welded frame scaffolding was not designed by a qualified person in that the manufacturer's instructions had not been followed. Employees were exposed to fall hazards when working from a plank supported by lateral supports for the end frames approximately 11 feet above the ground.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: a) At the site, on or about 08/15/2013, employees moved between scaffolds by climbing over a horizontal gap of approximately 22 inches. This exposed employees to a fall hazard of approximately 7 feet.
