29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 8/15/13, in room 109 of an old two story factory building being converted into apartments at 320 Grote St., Buffalo, NY: A "pancake type" Porter-Cable air compressor was used to supply air for power tools. 120 volt-ac power was supplied to the compressor via a series of extension cords and adapters. The green colored adapter in the series lacked a grounding prong. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: a) On or about 8/15/13, in room 109 of an old two story factory building being converted into apartments at 320 Grote St., Buffalo, NY: Electrical power was supplied to the Porter-Cable air compressor, a Ridgid Table Saw and a Ridgid Mitre Saw via a series of extension cords and adapters, the first of which was plugged into a 120 volt-ac wall receptacle at the end of the hallway. The green colored adapter in the series was plugged directly into this extension cord. This first extension cord in the series was No. 12 or larger and was spliced and taped. The repair was inadequate in that it created a large bulge in the cord, which did not retain the original properties of the outer sheath of the cord. NO ABATEMENT CERTIFICATION REQUIRED
