29 CFR 1910.120(j)(1)(vi): Prior to movement of drums or containers, all employees exposed during the transfer operation were not warned of the potential hazards associated with the contents of the drums or containers: (a) In the fuel blending process area, on August 14, 2013 and at times prior to, an employee was required to add drums of hazardous waste to the fuel blending process without being informed of the hazards associated with the contents of the containers, exposing employees to a fire hazard.

29 CFR 1910.120(j)(8)(iv): Bulking of hazardous wastes was permitted without a thorough characterization of the materials being completed: (a) In the area of the receiving pad, on August 15, 2013 and at times prior to, hazardous wasted being received at the facility were not characterized in a manner consistant with the facility's internal procedures prior to being added to the fuel blending process, exposing employees to a fire hazard.
