29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:          a) On or about 8/15/13, in room 109 of an old two story factory building being converted into apartments at 320 Grote St., Buffalo, NY: To supply power to a drill with a mixing paddle attachment used to mix grout for tile installation, a "bright green" extension cord was plugged into a green adapter in room 109 and run thru the hallway and into room 108 where an orange "Meredyk" adapter was plugged into it. An orange extension cord was plugged into the orange adapter and then a short 3-way extension cord was plugged into the orange cord and powered the drill. The green colored adapter in room 109 lacked a grounding prong which interrupted the path to ground to the drill.        NO ABATEMENT CERTIFICATION REQUIRED

The outer sheath of the orange extension cord in the series was frayed at the female end, exposing the conductors.    NO ABATEMENT CERTIFICATION REQUIRED
