Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Worksite:  Employees were performing work on a flat roof, and they were not protected from falling up to 15 feet 9 inches to the ground below by the use of appropriate fall protection systems and/or devices.     On or about 07/6/13, one of employees fell to the floor below through a hole on the roof and sustained serious injuries from the incident.
