29 CFR 1910.132(d)(1)(i): Employer did not select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment:  (a) On or about 08/27/2013, in basement, while employee used Easi Strip Supra to reclaim ink from screens. Employee did not wear gloves when handling screens emersed in the corrosive solution. Material Safety Data Sheet for Easi Strip Supra indicates the liquid is acidic with a pH of 2 to 3, users are to avoid skin and clothing contact, and that gloves are specified to be worn.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment:  (a) On or about 08/27/2013, for an employee working in the basement using Easi Strip Supra to reclaim ink from screens. Hand protection was not worn during this task. The Easi Stirip Supra is a corrosive cleaner with a pH of 2 to 3. A hazard assessment certification for personal protective equipment requirements for this task was not prepared.

Employees were not aware of the contents of employer's written Hazard Communication Program.

29 CFR 1910.134(c)(2)(i): The employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard") after determining that any voluntary use is permissible:  (a) On or about 08/27/2013, at establishment, for employees cleaning print screens while using 3M Model 8246 R95 particulate respirator on a voluntary basis while cleaning print screens with 2110 Low Odor Press Wash.
