29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Roof - On or about August 20, 2013 five employees using pneumatic air nail guns to install shingles were not wearing safety glasses. Employees were exposed to being struck by flying debris or nails.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) Roof - On or about August 20, 2013 eight employees were exposed to a fall hazard when installing waterproofing material on a 5:12 pitch roof with unprotected sides which were 7 feet 9 inches above floor level. Employees were not protected from falling to a lower level by the use of personal fall arrest system, safety net system or guardrail system.
