29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Roof of building, south wall, 1203 Western Avenue, Pittsburgh, Pennsylvania 15233: Observed on or about August 20, 2013, an employee working on the roof of a multistory commercial building was not protected from falling while steadying and moving a roll rig mobile anchor system located near the edge of the roof. The employee was exposed to a fall hazard of approximately 60 feet to the paved surface below.

29 CFR 1910.132(f)(1)(i): The employer did not provide training to each employee who is required by this section to use PPE on knowing when PPE is necessary: a) Roof of building, south wall, 1203 Western Avenue, Pittsburgh, Pennsylvania 15233: Observed on or about August 20, 2013, an employee working on the roof of a multistory commercial building was not protected from falling. The employer did not provide training to the employee in knowing when fall protection PPE is required. The employee was exposed to a fall hazard of approximately 60 feet to the paved surface below while moving and steadying a roll rig mobile anchor system.
