29 CFR 1910.305(b)(1)(i): Openings through which conductors entered cutout boxes, cabinets, or fittings were not effectively closed:    a)  At the worksite, on August 21, 2013, the conduit covering the 120 volt wiring entering the western most mixer panel was not completey closed, exposing employees to electrical shock.
