29 CFR 1926.602(d): Powered industrial truck operator training.  Note: The requirements applicable to construction work under this paragraph are identical to those set forth at ��1910.178(l) of this chapter:  29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  a. At the construction site located in Zanesville, Ohio the employer had an employee that operated a forklift that had not received the required formal training, practical training, and evaluation as required by 29 CFR 1910.178 (l)(2)(ii).
