Abatement certification was not received for the following violations:          a.    Citation 1 Item 1, and      b.    Citation 2 Item 1.

On September 11, 2013, Walker & Sons, Inc., was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:               a.       Citation 1 Item 1b, and              b.      Citation 1 Item 2b.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard of approximately 27 ft 8 in. and none of the fall protection systems specified above were used.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard of approximately 27 ft 8 in., and the employer did not maintain evidence of the required training. There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employer.
