29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  This violation occurred on August 19, 2013, at a job site located at 881 Starcreek Parkway, Dallas, TX; employees performing roofing work were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  This violation occurred on August 19, 2013, at a work place located at 881 Starcreek Parkway, Allen, TX; employees were exposed to fall hazards of approximately 10-feet by using ladders that did not extend 3-feet above the landing.
