29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Roof - Employees were observed on a roof with a slope of 6/12 while tearing off existing fiberglass shingles and were exposed to a 17 feet, 6 inch fall hazard, on or about 08/21/13.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.   For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) Roof - An employee was observed on a steep sloped roof 6/12 while tearing off existing fiberglass shingles and the lanyard was attached to the front of the body harness, on or about 08/21/13.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Exterior - Employees were observed using a portable extension ladder to gain access to the roof, 17 feet, 6 inches above the ground and the ladder extended 29 inches above the roof landing, on or about 08/21/13.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.   For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.
