29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about August 9, 2013, the employer did not ensure an employee cleaning up below the areas where roofing work was being conducted was in use of a protective helmet exposing the employee to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about August 9, 2013, the employer did not ensure employees reroofing the roof of a building were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about August 9, 2013, the employer did not ensure the side rails of three extension ladders located on the northeast side of a building, being used by employees to gain access to the upper level of a building, extended at least 3 feet above the upper landing exposing employees to a fall hazard.
