29 CFR 1917.48(i)(1): Conveyors were not stopped and their power sources were not locked out and tagged out during maintenance, repair, and servicing, except where the power was necessary for testing.    Location:  Marine Terminal located at 5500 Hall Street, St. Louis, MO  63147     On or about April 16, 2013 a mechanic was cleaning off the drive pulley of the C-3 Conveyor without locking out the energy sources.  The conveyor moved in the reverse direction under the weight of the coal remaining on the belt at the stacker reclaimer incline while the mechanic was shoveling coal away from the drive pulley.  The shovel was caught by the belt and pulley causing the mechanic's hand to be pinned between the pulley, belt, and shovel handle resulting in the amputation of two fingers.

29 CFR 1917.48(j)(3): Conveyors were operated without all overload devices, guards, and safety devices in place and operable:    The C3 Conveyor brake system was not operable on or about 04/16/2013 and did not restrain the loaded belt on the stacker reclaimer incline from reverse travel while a mechanic was removing wet coal from the west side of the drive pulley.
