29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) At the jobsite 6501 Veterans Parkway, Columbus, Georgia: The employer failed to ensure that employees were provided and/or utilized protective head equipment while working underneath a scaffold where tools, supplies, 5 gallon containers of brown mud for EIFS installation and other materials could could fall to the lower ground level. Employees were exposed to being struck by falling materials from the above EIFS work being performed, approximately 12-18 ft above the lower ground level.
