29 CFR 1910.268(c): The employer did not provide training in the various precautions and safe practices described in  this section and did not ensure that employees did not engage in the activities to which this section applies until such employees have received proper training in the various precautions and safe practices required by this section:    a) North Coast Wireless Communications, LLC at the worksite located at 20551 SR 58 in Wellington, Ohio: On or about August 7, 2013, the employer failed to provide training to assure safe climbing practices to prevent falls.  An employee who was not trained in fall protection was exposed to an approximate 100-foot fall hazard when climbing a communication tower.

29 CFR 1910.268(g)(1): Safety belts and straps were not provided and the employer did not ensure their use when work was performed at positions more than 4 feet above ground, on poles, and on towers, except as provided in paragraphs (n)(7) and (n)(8) of this section:    a) North Coast Wireless Communications, LLC at the worksite located at 20551 SR 58 in Wellington, Ohio: On  or about August 7, 2013, the employer failed to assure that adequate continuous fall protection was provided and used by employees who climbed communication towers.  A fall protection system that provided constant fall protection was not used when an employee climbed approximately 100 feet to maintain equipment on the communication tower.
