29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) University of Pennsylvania, 3601 Locust Walk, Philadelphia, PA - The employees were utilizing an energized (120 volts) electrical outlet box which was not being used according to its listing and labeling, thus exposing the employees to a shock/electrocution hazard. This violation was observed on or about August 23, 2013.
