29 CFR 1910.27(d)(1)(iv): Cages shall extend down the ladder to a point not less than 7 feet nor more than 8 feet above the base of the ladder, with bottom flared not less than 4 inches, or portion of cage opposite ladder shall be carried to the base:  (a) Poskin Feed Store, Grain Bin Area; Cages on fixed ladders did not extend down the ladder to a point not less than 7 feet nor more than 8 feet above the base of the ladder, such as, but not limited to:  (1) The outside leg ladder, and   (2) The 25,000 bushel bin ladder.

29 CFR 1910.303(b)(7):  Electrical equipment was not installed in a neat and workmanlike manner:  (a) Poskin Feed Store, ladder area of 30,000 bushel grain bin; The temporary wiring and splices energizing equipment on the 30,000 bushel bin was strung through and wrapped around structural members of the grain bins creating an open electrical hazard.

All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:  (a) Poskin Feed Store, ladder area of 30,000 bushel grain bin; The splice in the wiring for the temporary discharge auger was created by twisting the non-insulated ends of the copper conductors together.
