29 CFR 1910.23(a)(8)(ii): Every floor opening into which persons could accidentally walk was not guarded by a floor hole cover of standard strength and construction:   (a)  Foundry Area:  A floor opening located to the right of the furnace controls was not equipped with a cover.  The floor opening was 39 inches by 32 inches and measured 18 inches deep, as observed on September 5, 2013.
