29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite, 5804 Beacon Avenue, Pittsburgh, PA: On or about 08/27/13 an employee working on the edge of a steep roof approximately 25 feet above the ground was not protected from falling by the use of a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite, 5804 Beacon Avenue, Pittsburgh, PA: On or about 08/27/13 an employee was working approximately 25 feet above the ground without fall protection systems in place. The employer had not developed a training program on fall protection to ensure each person who has the potential exposure to fall hazards was able to recognize the hazards and what procedures and methods could be used to minimize exposure.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite, 5804 Beacon Avenue, Pittsburgh, PA: On or about 08/27/13 the portable ladder used to access the uppermost working level did not extend above the landing at least 3 feet. The employees used this ladder to access the work area on the roof that had a roof slope of 10/12 and the ladder only extended 2 feet above the landing.
