Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) Jobsite, 1660 Jefferson Avenue, Washington, PA 15301:  On or about August 27, 2013, four employees were working near the edge of the roof and they were not protected from falling nor was the company using a safety monitoring system.  The fall  hazard to the employees was approximately 8 1/2 feet to the ground.
