29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Jobsite, 1660 Jefferson Avenue, Washington, PA 15301: On or about August 27, 2013, four employees were working near the edge of the roof and they were not protected from falling nor was the company using a safety monitoring system. The fall hazard to the employees was approximately 8 1/2 feet to the ground.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) Jobsite, 1660 Jefferson Avenue, Washington, PA 15301: On or about August 27, 2013, the employer did not retrain all of the employees (who were working on the roof) in the dangers of working from a roof and the methods to minimize those hazards.
