29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    a.   On or about August 28, 2013, an employee replacing parking lot lights from the bucket of an aerial lift was exposed to a 25 feet fall hazard in that he was not wearing a harness and lanyard.
