29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:  (a) At the 7200 Block of Beechnut Street located in Houston, Texas, on or about 08/28/2013, employees working in an excavation over five feet in depth were unprotected from cave-ins and exposed to struck-by hazards.  All walls of the excavation were not sloped or benched at or less than 1:1.
