29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energy, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   a.  1750 Lakes Parkway, Lawrenceville, GA (Shop Area) - The employees were exposed to sudden energization or start-up of multi-energy source machines.   The employer did not develop and implement a lockout/tagout program with machine specific written procedures for locking out/tagging out the multi-energy machines and hardwired electrical machines to safe guard employees performing services and/or maintenance on machinery or equipment, on or about 08/28/2013.           a. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     a.  1750 Lakes Parkway, Lawrenceville, GA (Shop Area) -  Employees were exposed to chemical hazards.  The employer did not develop and implement a written hazard communication program for employees exposed to hazardous chemicals on the jobsite.  The employees/operators are required to handle the chemicals used for the press machines. The chemicals and/or solvents include, but are not limited to, developer solution, ink, quick wash and printing press ink cleaners, on or about 08/28/2013.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a.  1750 Lakes Parkway, Lawrenceville, GA (Shop Area) - Employees were exposed to chemical hazards.  Employees were not provided with training and information on the types of hazardous chemicals/solvents used at the workplace for the press machines such as, but not limited to, developer solution, ink, quick wash and printing press ink cleaners.  Employees were not trained to be familiar with the Material Safety Data Sheets for the chemicals and metals, on or about 08/28/2013.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   a.  1750 Lakes Parkway, Lawrenceville, GA (Shop Area) - Employees were  exposed to struck-by hazards.  The employer failed to provide re-training for the designated forklift operators to ensure that the designated operators had retained the knowledge and skills needed to operate the powered industrial trucks. The last forklift training was greater than five years ago, on or about 08/28/2013.
