29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  (a) Shop: Employees were exposed to electrical hazards as there were no machine specific Lockout Tagout procedures for machinery that was hardwired in, and had more than one source of energy, for equipment such as, but not limited to, the Hyperthem Plasma Cutter.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  (a) Shop/Manufacturing:  Employees were exposed to fire hazards as an oxygen cylinder and an acetylene cylinder were stored together. (a) Shop/Manufacturing: Employees were exposed to electrical hazards as the Millermatic 251 Wire Welder had a cut in the insulation for the electrical power cord.  (b) Shop/Manufacturing: Employees were exposed to electrical hazards as the Schumacher Fast Charger had a cut in the insulation for the electrical power cord.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
