29 CFR 1926.100(a):  Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At the worksite:  On or about August 29, 2013, and at times prior thereto, the employer allowed employees to conduct bridge construction activities without head protection with overhead hazards present.  Employees were exposed to struck-by hazards from falling and flying objects.         Sewell Brothers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CRF 1926.100(a), which was contained in OSHA inspection number 314927682, citation number 01, item number 002 and was affirmed as a final order on March 8, 2011 with respect to a workplace located at Northeast 10th and Anderson Road, Choctaw, Oklahoma.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:        At the worksite:  On or about August 29, 2013, and at times prior thereto, the employer allowed employees to conduct bridge construction activities without a fall protection system to protect them from falls from heights greater than 6 feet (1.8 m).  Employees were exposed to the hazard of fall from heights.

Employees were exposed to the hazards of impalement.
