29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:   Well site:  On or about August 27, 2013, an employee operating a transport tanker was loading the tanker with flammable liquid hydrocarbons from an oil/gas well tank battery.  Ignition sources as a cell phone, bluetooth headset, and potential of static electricity were present.  This exposed the employee to the hazards of explosion and/or fire.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  Well site:  On or about August 27, 2013, and times prior to, an employee operating a tanker transport was loading flammable liquid hydrocarbons into the tanker trailer.  The employee was not wearing a flame resistant shirt.  This exposed the employee to the hazards of explosion and/or fire.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Well site:  On or about August 27, 2013, an employee operating an transport tanker was loading the tanker with oil/gas well liquid hydrocarbons from a well tank battery.  The employee was not wearing any eye protection.  This exposed the employee to the hazard of being struck in the eyes with the hydrocarbons or debris generated by the activity.
