29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Location:  9706 Inter Ocean Drive, Cincinnati, OH 45246  a) On or about 6 Aug, 2013, the employer did not the guard the point of operation on a press brake.  An employee's hand was crushed, resulting in the loss of a finger tip, when the employee's hand was able to enter the point of operation while she was operating the press.     In accordance with 29 CFR 1903.19(d), abatement certifiaction is required for thiss violation(using the  CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to the purchase or repair of the equipment, photographic or video evidence, or written records.
