29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer did not establish and implement a written respiratory protection program with worksite-specific procedures. The program was not updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer did not include in the program the following provisions of this section, as applicable.    This violation most recently occurred on August 29, 2013, at a workplace located at 5101 Frye Road, Irving, TX 75061; where at least one (1) employee was required to use a N95 respirator, exposed to potential reduction of pulmonary function, elevations in heart rate, blood pressure, and/or respiratory rate hazards.
