29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee. On or about 9/3/13, at the Winder job site, employees using fall protection equipment were not properly anchored off using anchorage points that would support 5,000 lbs per employee. On one house 2 employees used the same Guardian anchorage device and on another house employees wrapped the anchorage rope around a roof joist and back into itself as a means of anchor point. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On or about 9/3/13, at the Winder job site, employees were using anchorage points in a fall protection system that was not installed and used under the direction of a qualified person. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On 9/3/13, the employer had not created a fall protection training program for employees working on 1 story buildings, replacing roof shingle systems that were exposed to fall hazards up to 14' on a 4 in 12 slope. Employees were using fall protection equipment but had not been trained in the proper use of that equipment. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
