29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  On or about September 04, 2013, at this work site, an employee was exposed to the hazards of amputation on a fixed water pump with an unprotected shaft measuring approximately 3 inches long by 2 and 3/4 inches in diameter while doing routine maintenance, such as but not limited to changing and adjusting of belts and filling reservoirs with hydraulic fluids.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):  On or about September 04, 2013, at this work site, an employee was exposed to the hazards of amputation on 2 sections of a fixed water pump without machine guards while doing routine maintenance, such as but not limited to, changing belts, adjusting work pressures and filling reservoirs with hydraulic fluids:  (a) Left hand side of the fixed water pump system has an unprotected belt system that measures approximately 3 inches wide by 6 inches in length.  (b) Right hand side of the fixed water pump system has  unprotected belt system that measures approximately 1/2 on an inch wide by 13 inches in length.
