29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. a) On or about September 4, 2013, and at times prior to, an employee stands on a table platform to remove piled up plastics without fall protection, exposing employee to a fall hazard of approximately 4.5 feet to the ground. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) On or about September 4, 2013, and at times prior to,employees operated a forklift that did not have a functioning back-up alarm or a functioning horn. Employees used the forklift to move materials/plastics to the baler and bales for shipping in the workshop, exposing employees to struck-by and crush hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.
