Employees performing framing and carpentry work at this multi-family residential project worked from wood pole scaffold platforms greater than 10 feet above ground level without fall protection.

At this job site personal fall arrest equipment was not in use; neither guardrails nor nets were installed. Employees were exposed to falls from the second-story decks.
