29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:      Throughout this multi-family residential dwelling project, employees performing framing and carpentry worked and walked on job made, single plank scaffold platforms which were not 18 inches wide. Employees were exposed to falls greater than 10 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Employees performing framing and carpentry work at this multi-family residential project worked from wood pole scaffold platforms greater than 10 feet above ground level without fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     At this multi-family residential dwelling project, employees installing decking on the roof of the buildings under construction did not have fall protection to prevent a fall to ground level. At this job site personal fall arrest equipment was not in use; neither guardrails nor nets were installed. Employees were exposed to falls from the second-story decks.
