29 CFR 1910.305(a)(4)(v): Conductors within 7 feet from the floor, thus considered exposed to physical damage, were not protected:    a) On or about 08/28/13, at #8 Estate Diamond, Frederiksted, VI, in the conveyor area, the electrical conductors of the Turn Head machine were not protected from damage.    b) On or about 08/28/13, at #8 Estate Diamond, Frederiksted, VI, in the cuber machine area, the electrical conductors of the machine were not protected from damage.    c) On or about 08/28/13, at #8 Estate Diamond, Frederiksted, VI, in the maintenance shop area, the electrical conductors of the electrical outlet/light swtich were not protected from damage.    PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear:    On or about 08/28/13, at #8 Estate Diamond, Frederiksted, VI, in the maintenance shop, an electrical outlet box was not covered.    PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced:    On or about 08/28/13, at #8 Estate Diamond, Frederiksted, VI, in the maintenance shop area, the electrical cord of the press drill was spliced and repaired with a different electrical cord.
