29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: AVD i1/ is amend to read following as stated in SSA. 1/ On or about 08/12/2013, at the jobsite at 373 Sunset Estates, Saint Clair, MO, employees installing and standing on trusses were not provide with conventional fall protection and the employer did not have a fall protection plan that complied with 29 C.F.R. 1926.502 (k). 2/ On or about 08/12/2013, at the jobsite at 373 Sunset Estates, Saint Clair, MO, two employees standing on truss members that were 12' high and were not provided conventional fall protection. The total fall distance from the truss and from the top of the outer wall was approximately 30 feet.

29 CFR 1926.503(a)(1): The employer does not ensure that a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about 08/12/2013, at the jobsite at 373 Sunset Estates, Saint Clair, MO, an employee was exposed to an approximately 19 feet 6 inches fall. Fall protection was not utilized and the employer failed to provide the employee with fall protection training.

29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: On or about 08/12/2013, at the jobsite at 373 Sunset Estates, Saint Clair, MO, four employees were working off of a garage wall installing trusses, when two of them broke, resulting in three of the employees falling to the ground with multiple injuries that required hospitalization. The incident was reported on 08/21/2013.
