29 CFR 1910.151(a): Medical personnel were not readily available for advice and consultation on matters of plant health.  On or about September 3, 2013 and times prior thereto, employees of Ponder Well Service, Inc. performed servicing on crude oil well Wepfer A1 with a Franks mobile 300 workover rig at or near latitude North 33.4035 and longitude West 93.13903 in McNeil, Arkansas.  The company did not have medical personnel readily available for consultation to provide advice to prevent employee heat-related illness and guidance during a medical emergency involving an employee suffering from heat stroke.

29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:  On or about September 3, 2013 and times prior thereto, employees of Ponder Well Service, Inc. performed services on the Wepfer A1 well with the use of a Franks 300 mobile workover rig at or near latitude North 33.4035 and longitude West 093.13903.  There was not a person adequately trained to render first aid when there was not a medical facility for the treatment of injured employees near proximity to the workplace.

On or about September 3, 2013 and times prior thereto, Ponder Well Service, Inc. failed to report to OSHA a work-related death of an employee at the Wepfer A1 well located at or near Latitude coordinate North 33.4035 and Longitude coordinate West 93.13903 in McNeil, Arkansas.
